Ana Paula Martins dos Reis Arez is Assistant Researcher at the Institute of Hygiene and Tropical Medicine, University Nova de Lisboa. She works in the area of Health and Biological Sciences, mainly in research related to malaria and other parasitic diseases. She has published 39 articles in specialized journals and papers in proceedings of 9 events. It has 60 items of technical production. In their professional activities interacted with 200 colaborators in co-authorship of scientific papers. Between 1993 and 2010 has participated in 10 research projects and coordinated 2. Presently is team member of 6 projects, coordinating 2. In her DeGóis curriculum, most frequent keywords are: malaria, diversity, Anopheles, transmission, molecular markers, Plasmodium falciparum, Plasmodium, Human susceptibility/resistance, drug resistance e mixed infections. Ana Paula Martins dos Reis Arez é Investigadora Auxiliar no Instituto de Higiene e Medicina Tropical da Universidade Nova de Lisboa. Concluiu a Agregação - em 2011. Atua na área de Ciências da Saúde e Ciências Biológicas, essencialmente em investigação relacionada com malária e outras doenças parasitárias. Publicou 39 artigos em revistas especializadas e 9 trabalhos em atas de eventos. Possui 60 itens de produção técnica. Nas suas atividades profissionais interagiu com 200 colaboradores em coautorias de trabalhos científicos. Entre 1993 e 2010 participou em 10 projetos de investigação, sendo que coordenou 2 destes. Atualmente participa em 6 projetos de investigação, sendo que coordena 2 destes. 1. Malaria parasite diversity - infection and transmission dynamics
Objectivos»Goals: Parasite epidemiology, especially malaria, transmission dynamics and interactions and ecological relationships between different parasites co-infecting the same host have always been my main areas of interest . Within this scope, I have conducted several research projects in malaria endemic areas, namely Guinea-Bissau, Cape Verde, Mozambique and Equatorial Guinea (on-going project). Presently, main objectives are 1) To characterise circulating populations - Plasmodium species and genotypes, and describe interactions between them and with both hosts (human and mosquitoes), 2) To analyse selective pressures acting upon drug resistance genes and inferring on origin and spread of mutations through the comparison between flanking microsatellite sequences to drug resistance associated genes and neutral loci, 3) To analyse host factors such as Duffy blood group and mosquito immune related genes and to infer implications for malaria infection/transmission, 4) To investigate the parasite blood-stage dynamics and what the clinical implications are when mixed infections occur through a new modelling approach and 5) To analyse the effect of environmental conditions and transmission levels on the above aspects..

2. Host-Parasite interactions - factors of human host susceptibility/resistance to infection/disease
Objectivos»Goals: The interesting and unexpected observations obtained in Cape Verde led to a “new” line of research investigating which human host factors may affect the susceptibility or resistance to malaria (infection and/or disease). Consequently, from approximately 2005 onwards, although continuing my former line of research, I have developed this new one, which is much more molecular/cellular and experimentally-based. So far, encouraging results have been obtained and new plans and projects for future work are being implemented. Presently, we are focusing how disorders on the glycolytic and pentose phosphate pathways of the red blood cell may affect Plasmodium infection, studying the effect of 3 enzymatic deficiencies (G6PD,PK,TPI) on parasite development. Main objectives are: 1) To assess infection dynamics in enzyme-deficient and healthy blood both in in vitro as in vivo experimental models, 2) To analyse the proteomic profile of non-infected and infected erythrocytes (from cultures and rodent infections) as well as of parasites (isolated from both deficient and normal host cells), 3) To determine frequencies of polymorphisms of g6pd, pklr and tpi genes in African populations and 4) To investigate associations of individual haplotypes and malaria infection/ clinical status..

Recent reports of Plasmodium vivax infections, the most widely distributed species of human malaria, show that this parasite is evolving and adapting, becoming not only more aggressive but also more frequent in countries where it was not present in the past, becoming, therefore, a major source of concern. Thus, it is extremely important to perform new studies of its distribution in West and Central Africa, where there are few reports of its presence, due to the high prevalence of Duffy-negative individuals. The aim of this study was to investigate the presence of P. vivax in Angola and in Equatorial Guinea, using blood samples and mosquitoes. The results showed that P. vivax seems to be able to invade erythrocytes using receptors other than Duffy, and this new capacity is not exclusive to one strain of P. vivax, since we have found samples infected with two different strains: VK247 and classic. Additionally we demonstrated that the parasite has a greater distribution than previously thought, calling for a reevaluation of its worldwide distribution.

"SLC40A1 Q248H allele frequencies and associated SLC40A1 haplotypes in three West African population samples", Annals of human biology 38, 3: 378 - 381.
Background: Ferroportin is a transmembrane protein responsible for iron export from enterocytes and macrophages. Mutation c.744G ¿ T (Q248H), located in exon 6 of the ferroportin gene SLC40A1, is found as a polymorphism in populations of African origin. This mutation has been extensively analysed in African-Americans, but poorly studied in native African populations. Aim: To increase information about Q248H mutation frequency in native sub-Saharan populations examining three West African populations. Subjects and methods: Samples from S. Tomé e Príncipe (n = 115), Angola (n = 156) and Republic of Guinea (n = 170) were analysed for Q248H mutation and for two polymorphisms, IVS1( - 24)G ¿ C and microsatellite (CGG)n, using standard molecular methodology. Results: The estimated frequencies of Q248H allele were 2.2% in S. Tomé e Príncipe, 3.5% in Angola and 4.1% in Republic of Guinea. Analysis of polymorphisms IVS1( - 24)G ¿ C and (CGG)n showed mutation allele c.744T to be strongly associated with haplotype IVS1( - 24)G/(CGG)7. Conclusions: This study confirmed the presence of Q248H mutation at polymorphic frequencies in three native sub-Saharan populations. Analysis of two additional markers in the same gene support a single origin of the mutant allele c.744T in the haplotype background IVS1( - 24)G/(CGG)7.

To evaluate the risk of transmission of vector-borne diseases, regular updates of the geographic distribution of insect vectors are required. In the archipelago of Cape Verde, nine mosquito species have been reported. Of these, four are major vectors of diseases that have been present in the archipelago: yellow fever, lymphatic filariasis, malaria and, currently, an outbreak of dengue. In order to assess variation in mosquito biodiversity, we have carried out an update on the distribution of the mosquito species in Cape Verde, based on an enquiry of 26 unpublished technical reports (1983–2006) and on the results of an entomological survey carried out in 2007. Overall, there seems to be a general trend for an expansion of biological diversity in the islands. Mosquito species richness was negatively correlated with the distance of the islands from the mainland but not with the size of the islands. Human- and/or sporadic climatic-mediated events of dispersal may have contributed to a homogenization of species richness regardless of island size but other ecological factors may also have affected the mosquito biogeography in the archipelago. An additional species, Culex perexiguus, was collected for the first time in the archipelago during the 2007 survey.

In the Democratic Republic of East Timor, Plasmodium falciparum and Plasmodium vivax malaria coexist, but limited information is available about the latter species. Consequently, the prevalence of P. vivax and of its corresponding antifolate resistance-associated mutations in the pvdhfr and pvdhps genes was assessed here. Blood samples were collected from 650 individuals distributed among six districts, over two different periods, by either passive case detection (PCD) or active case detection (ACD). As expected, malaria was over-represented in the PCD sample (26% PCD vs 5% ACD), because the infection increases medical care seeking. Additionally, the relative frequency of P. vivax infections in symptomatic individuals (37%) was twice as high as the one in the asymptomatic sampling group (18%), suggesting that that this parasite is accounting for a significant proportion malaria-attributed morbidity. The frequency of specific sulfadoxine-pyrimethamine resistance-associated mutations genes was ascertained in P. vivax positive samples by PCR-RFLP. Although no mutants were detected in codons 383 and 553 of pvdhps, 48%, 76% and 82% of P. vivax-infected samples harbored the dhfr 33L, 58R and 117N mutations, respectively. Additionally, the frequency of parasites carrying both pvdhfr 58R and 117N mutant alleles accounted for a third of all genotypes analyzed, most likely due to inadvertent SP use in the past. In conclusion, evidence-based information is provided to promote optimized drug deployment and limit the evolution of resistance to antifolate resistance in P. vivax from East Timor.

Objective: To investigate malaria prevalence and socio-economic conditions in East Timor. Methods: Blood samples were collected from 650 individuals distributed among six districts in East Timor. 434 and 216 individuals were sampled by passive and active case detection, respectively (PCD, ACD). Results: The results showed that the plasmodium infection prevalence was 18.9% with predominance of Plasmodium falciparum(P. falciparum) infections (60.2%). The majority of cases were detected in children between 2-14 years old in both PCD and ACD. Participants residing in Same and Lospalos were at a higher risk of malaria. The percentage of infections associated to the lack of bed net use was twice as high as users (25.2% vs 12.5%). The prevalence of malaria among participants who lived in brick/cement houses (8.3%) was two times less than those who lived in other types of houses (3.7%). There was a significantly lower prevalence of infection detected in individuals living in complete housing constructions with absence of domestic animals in or near the houses. Conclusions: The major risk factors.

Background. Malaria in humans is caused by apicomplexan parasites belonging to 5 species of the genus Plasmodium. Infections with Plasmodium ovale are widely distributed but rarely investigated, and the resulting burden of disease is not known. Dimorphism in defined genes has led to P. ovale parasites being divided into classic and variant types. We hypothesized that these dimorphs represent distinct parasite species. Methods. Multilocus sequence analysis of 6 genetic characters was carried out among 55 isolates from 12 African and 3 Asia-Pacific countries. Results. Each genetic character displayed complete dimorphism and segregated perfectly between the 2 types. Both types were identified in samples from Ghana, Nigeria, São Tomé, Sierra Leone, and Uganda and have been described previously in Myanmar. Splitting of the 2 lineages is estimated to have occurred between 1.0 and 3.5 million years ago in hominid hosts. Conclusions. We propose that P. ovale comprises 2 nonrecombining species that are sympatric in Africa and Asia. We speculate on possible scenarios that could have led to this speciation. Furthermore, the relatively high frequency of imported cases of symptomatic P. ovale infection in the United Kingdom suggests that the morbidity caused by ovale malaria has been underestimated. .

The genetic component of susceptibility to malaria is both complex and multigenic and the better-known protective polymorphisms are those involving erythrocyte-specific structural proteins and enzymes. In vivo and in vitro data have suggested that pyruvate kinase deficiency, which causes a nonspherocytic haemolytic anaemia, could be protective against malaria severity in humans, but this hypothesis remains to be tested. In the present study, we conducted a combined analysis of Short Tandem Repeats (STRs) and Single Nucleotide Polymorphisms (SNPs) in the pyruvate kinase-encoding gene (PKLR) and adjacent regions (chromosome 1q21) to look for malaria selective signatures in two sub-Saharan African populations from Angola and Mozambique, in several groups with different malaria infection outcome. A European population from Portugal, including a control and a pyruvate kinase-deficient group, was used for comparison. Data from STR and SNP loci spread along the PKLR gene region showed a considerably higher differentiation between African and Portuguese populations than that usually found for neutral markers. In addition, a wider region showing strong linkage disequilibrium was found in an uncomplicated malaria group, and a haplotype was found to be associated with this clinical group. Altogether, this data suggests that malaria selective pressure is acting in this genomic region.

Malaria has occurred in the Cabo Verde archipelago with epidemic characteristics since its colonization. Nowadays, it occurs in Santiago Island alone and though prophylaxis is not recommended by the World Health Organization, studies have highlight the prospect of malaria becoming a serious public health problem as a result of the presence of antimalarial drug resistance associated with mutations in the parasite populations and underscore the need for tighter surveillance. Despite the presumptive weak immune status of the population, severe symptoms of malaria are not observed and many people present a subclinical course of the disease. No data on the prevalence of sickle-cell trait and red cell glucose-6-phosphate dehydrogenase deficiency (two classical genetic factors associated with resistance to severe malaria) were available for the Cabo Verde archipelago and, therefore, we studied the low morbidity from malaria in relation to the particular genetic characteristics of the human host population. We also included the analysis of the pyruvate kinase deficiency associated gene, reported as putatively associated with resistance to the disease. Allelic frequencies of the polymorphisms examined are closer to European than to African populations and no malaria selection signatures were found. No association was found between the analyzed human factors and infection but one result is of high interest: a linkage disequilibrium test revealed an association of distant loci in the PKLR gene and adjacent regions, only in non-infected individuals. This could mean a more conserved gene region selected in association to protection against the infection and/or the disease.

A compreensão do complexo ciclo de vida da malária tem aumentado muito nos últimos anos mas, apesar de décadas de pesquisa e luta contra a doença, esta continua a ser um dos principais problemas de saúde pública, especialmente nas áreas mais pobres do planeta. Devido à sua elevada prevalência em certas regiões do globo, desde há cerca de 10 mil anos, a malária tem exercido uma pressão seletiva muito forte no genoma humano. A componente genética de suscetibilidade ao parasita é complexa, com uma variedade de polimorfismos a influenciar a patogénese e resposta do hospedeiro, e um dos desafios na luta contra esta doença é avaliar estes determinantes de suscetibilidade e decifrar os mecanismos envolvidos para utilizá-los como novos alvos para fármacos ou vacinas. Entre os polimorfismos genéticos humanos descritos como protetores contra a malária, os mais comuns e melhor caracterizados envolvem proteínas estruturais específicas (tais como as hemoglobinas S e C, as talassémias, o antigénio Duffy e o grupo sanguíneo O) e enzimas eritrocitárias (como a deficiência de glucose-6-fosfato desidrogenase, e, mais recentemente descrita, a deficiência de piruvato cinase). Esta pequena revisão aborda estas variantes genéticas e discute alguns dos resultados controversos obtidos, assim como os mecanismos que podem justificar esta proteção.

Background In response to chloroquine (CQ) resistance, the policy for the first-line treatment of uncomplicated malaria in the Democratic Republic of East Timor (DRET) was changed in early 2000. The combination of sulphadoxine-pyrimethamine (SP) was then introduced for the treatment of uncomplicated falciparum malaria. Methods Blood samples were collected in two different periods (2003–2004 and 2004–2005) from individuals attending hospitals or clinics in six districts of the DRET and checked for Plasmodium falciparum infection. 112 PCR-positive samples were inspected for genetic polymorphisms in the pfcrt, pfmdr1, pfdhfr and pfdhps genes. Different alleles were interrogated for potential associations that could be indicative of non-random linkage. Results Overall prevalence of mutations associated with resistance to CQ and SP was extremely high. The mutant form of Pfcrt (76T) was found to be fixed even after five years of alleged CQ removal. There was a significant increase in the prevalence of the pfdhps 437G mutation (X2 = 31.1; p = 0.001) from the first to second survey periods. A non-random association was observed between pfdhfr51/pfdhps437 (p = 0.001) and pfdhfr 59/pfdhps 437 (p = 0.013) alleles. Conclusion Persistence of CQ-resistant mutants even after supposed drug withdrawal suggests one or all of the following: local P. falciparum may still be inadvertently exposed to the drug, that mutant parasites are being "imported" into the country, and/or reduced genetic diversity and low parasite transmission help maintain mutant haplotypes. The association between pfdhfr51/pfdhps437 and pfdhfr 59/pfdhps 437 alleles indicates that these are undergoing concomitant positive selection in the DRET. .

Population samples from Angola, Mozambique, and S. Tomé e Príncipe were screened for the TPI gene promoter variants -5A¿G, -8G¿A and -24T¿G. Three haplotypes were identified in the three populations: the haplotype -5A-8G-24T (average frequency 65.3%) and two less common haplotypes -5G-8G-24T (average frequency 24.7%) and -5G-8A-24T (average frequency 10.0%). A population sample from Central Portugal showed the haplotype -5A-8G-24T in 139 chromosomes and one subject heterozygous for haplotype -5G-8A-24G. The exact test of sample differentiation among three groups of malaria-infected individuals classified according to the severity of the disease showed no significant differences. We confirmed TPI gene diversity in sub-Saharan Africa, but we could not detect any association between TPI promoter variation and a malarial protective effect. Larger scale epidemiological studies are thus required to clarify this putative mechanism of natural host defense against this worldwide public health problem.

In the malaria vector Anopheles gambiae Giles sensu stricto, two molecular forms denoted M and S are considered units of incipient speciation within this species. Very low hybrid frequencies and signiÞcant genetic differentiation have been found in sympatric M- and S-form populations. We studied the molecular form composition and the degree of genetic differentiation at 15 microsatellites in two samples of An. gambiae collected in two consecutive years from Bissau, Guinea Bissau. High frequencies of M/S hybrids (19Ð24%) were found in this area. Coincidently, very low levels of genetic differentiation were detected between forms when analysis involved microsatellites mapped at chromosome-3 (mean Fst, 0.000Ð0.002). The single exception was the X-linked AGXH678, for which high differentiation was measured (Fst, 0.158Ð0.301). This locus maps near the centromere of chromosome X, a low recombination region in which selection is likely to promote divergence between M and S forms. These results strongly suggest that the degree of isolation between M and S forms, considered the units of incipient speciation within An. gambiae, is not homogenous throughout the species distribution range.

Malaria is one of the main human public health problems in the tropical world and is possibly becoming an emerging disease too in regions where it has been controlled. It has been an excellent model in the area of molecular studies, with scientific validation of techniques, application of data mainly in studies of parasite diversity and information on a number of different aspects associated with infection and disease. The transfer of the gathered knowledge and experience in malaria to other infections is of great use and we briefly review a number of molecular markers, methodologies and techniques mostly used for Plasmodium detection, as well as identification or characterization of parasite populations. Selection of appropriate techniques depends on the questions raised and the studies’ objectives – the antigen-coding genes, microsatellite loci and drug-resistance associated markers being the three most analysed classes of markers. The need of validation and standardization of laboratory protocols is addressed and discussed as it may determine the comparison of data between different studies and laboratories, with relevance in field-collected samples or studies.

Plasmodium malariae, the parasite responsible for quartan malaria, is transmitted in most areas of malaria endemicity and is associated with significant morbidity. The sequence of the gene coding for the enzyme dihydrofolate reductase-thymidylate synthase (DHFR-TS) was obtained from field isolates of P. malariae and from the closely related simian parasite Plasmodium brasilianum. The two sequences were nearly 100% homologous, adding weight to the notion that they represent genetically distinct lines of the same species. A survey of polymorphisms of the dhfr sequences in 35 isolates of P. malariae collected from five countries in Asia and Africa revealed a low number of nonsynonymous mutations in five codons. In five of the isolates collected from southeast Asia, a nonsynonymous mutation was found at one of the three positions known to be associated with antifolate resistance in other Plasmodium species. Five isolates with the wild-type DHFR could be assayed for drug susceptibility in vitro and were found to be sensitive to pyrimethamine (mean 50% inhibitory concentration, 2.24 ng/ml [95% confidence interval, 0.4 to 3.1]).

Abstract Background Malaria has come near eradication at archipelago of Cabo Verde in 1970. Infections are now only observed in Santiago, where outbreaks occur. In these islands, malaria is considered by the international community as being of limited risk and, therefore, no prophylaxis is recommended. Since the understanding of factors that determine malaria outbreaks are crucial for controlling the disease, the present study aimed to investigate if the malaria infections observed in Santiago Island are maintained in isolated foci and in asymptomatic individuals. Methods The occurrence of asymptomatic carriers in villages with history of malaria as well as the level of exposure of these populations were investigated using PCR and serological analyses. Results Results indicate that malaria is maintained as asymptomatic and sub-patent infections and that the majority of the circulating parasite populations harbour chloroquine-resistant mutations. Conclusion These observations highlight the alarming prospect of malaria to become a serious public health problem and underscore the need for a tighter surveillance. .

We compared the distribution patterns of individual Plasmodium species and mixed-species infections in two geographically close endemic areas, but showing environmental differences. Comparisons concerned circulating Plasmodium infections in both human and mosquito vector populations in the dry and wet seasons, at a micro-epidemiological level (households). Both areas revealed a very high overall prevalence of infection, all year-round and in all age groups. Plasmodium falciparum was the predominant species, being found in the vast majority of infected individuals regardless of the presence of other species. Plasmodium malariae and Plasmodium ovale occurred almost exclusively in mixed infections. Seasonal variation in P. malariae prevalence was observed in one area but not in the other. A decrease in P. malariae prevalence concurred with a marked increase of P. falciparum prevalence. However this was strongly dependent on age and when analysing infections at the individual level, a different pattern between co-infecting species was unveiled. Regarding transmission patterns, in both areas, P. falciparum gametocytes predominated in single infections regardless of age and P. malariae gametocyte carriage increased when its overall prevalence decreased.

Summary An entomological study on vectors of malaria and their relative contribution to Plasmodium falciparum transmission in the semi-urban area of Ifakara, south-eastern Tanzania, was conducted. A total of 32 houses were randomly sampled from the area and light trap catches (LTC) performed in one room in each house every 2 weeks for 1 year. A total of 147 448 mosquitoes were caught from 789 LTC; 26 134 Anopheles gambiae s.l., 615 A. funestus, 718 other anophelines and 119 981 culicines. More than 60% of the total A. gambiae s.l. were found in five (0.6%) LTCs, with a maximum of 5889 caught in a single trap. Of 505 A. gambiae s.l. speciated by polymerase chain reaction, 91.5% were found to be A. arabiensis. Plasmodium falciparum sporozoite enzyme-linked immunosorbent assay tests were performed on 10 108 anopheles mosquitoes and 39 (0.38%) were positive. Entomological inoculation rate (EIR) estimates were generated using a standard method and an alternative method that allows the calculation of confidence intervals based on a negative binomial distribution of sporozoite positive mosquitoes. Overall EIR estimates were similar; 31 vs. 29 [95% confidence interval (CI): 19, 44] infectious bites per annum, respectively. The EIR ranged from 4 (95% CI: 1, 17) in the cool season to 108 (95% CI: 69, 170) in the wet season and from 54 (95% CI: 30, 97) in the east of the town to 15 (95% CI: 8, 30) in the town centre. These estimates show large variations over short distances in time and space. They are all markedly lower than those reported from nearby rural areas and for other parts of Tanzania.

Clinical treatment failures of the hydroxynaphthoquinone atovaquone or its combination with proguanil (Malarone™) in Plasmodium falciparum malaria has been recently documented. These events have been associated to single nucleotide polymorphisms (SNPs) in the parasite cytochrome b gene (cytb). In this report we describe a set of nest PCR-RFLP methods developed for the fast detection of all known cytb mutations associated to resistance to these drugs. The methods were successfully applied for the analysis of phenol–chloroform extracted DNA samples from patients not cured by Malarone™, and from an established parasite clone. Further, the protocol for the detection of the A803C mutation was applied to 164 DNA field samples extracted through crude methanol-based protocols, originated from several malaria settings. The PCR-RFLP methods here presented can be used as a valuable for the clinical detection and study of Malarone™ and atovaquone P. falciparum resistance.

We studied malaria transmission by comparing parasite populations in humans and mosquito vectors at the household level. Blood samples were collected from all inhabitants for microscopic detection of gametocytes and polymerase chain reaction analysis. The next morning, blood-fed resting mosquitoes were collected inside the bed nets used by the individuals surveyed the previous afternoon. After 8 days of maintenance, mosquitoes were dissected, and midguts and salivary glands were recovered for polymerase chain reaction analysis. Results showed that parasite distribution was the same in the 2 hosts when compared at each household but was different when whole populations were analyzed. Different associations of Plasmodium species seem to occur in humans (Plasmodium falciparum/Plasmodium malariae) and mosquitoes (P. falciparum/Plasmodium ovale). Regarding P. falciparum infections, a higher proportion of single-genotype infections and less allele diversity are observed in mosquitoes than in humans. .

Genetic diversity of malaria parasites represents a major issue in understanding several aspects of malaria infection and disease. Genotyping of Plasmodium falciparum infections with polymerase chain reaction (PCR)-based methods has therefore been introduced in epidemiological studies. Polymorphic regions of the msp1, msp2 and glurp genes are the most frequently used markers for genotyping, but methods may differ. A multicentre study was therefore conducted to evaluate the comparability of results from different laboratories when the same samples were analysed. Analyses of laboratory-cloned lines revealed high specificity but varying sensitivity. Detection of low-density clones was hampered in multiclonal infections. Analyses of isolates from Tanzania and Papua New Guinea revealed similar positivity rates with the same allelic types identified. The number of alleles detected per isolate, however, varied systematically between the laboratories especially at high parasite densities. When the analyses were repeated within the laboratories, high agreement was found in getting positive or negative results but with a random variation in the number of alleles detected. The msp2 locus appeared to be the most informative single marker for analyses of multiplicity of infection. Genotyping by PCR is a powerful tool for studies on genetic diversity of P. falciparum but this study has revealed limitations in comparing results on multiplicity of infection derived from different laboratories and emphasizes the need for highly standardized laboratory protocols.

We present the first parasitological, molecular and longitudinal analysis of an isolated outbreak of malaria. This outbreak occurred on Santiago Island (Republic of Cabo Verde), a region where malaria is hypoendemic and controlled, and thus the population is considered non-immune. Blood samples were collected from the inhabitants over 1 month and during cross-sectional surveys in the following year. The presence and nature of the parasites was determined by PCR. Plasmodium falciparum was the only species detected. Genetic analysis revealed that the circulating parasites were genetically homogeneous, and probably clonal. Gametocytes were found throughout this period. Our data suggest that this represented a focal outbreak, resulting in the infection of at least 40% of the villagers with a clonal parasite line. Thus, P. falciparum infections can persist for at least 1 year in a substantial proportion (10%) of the hosts. Implications for malaria control and the interpretation of epidemiological data are discussed.

Polymerase chain reaction (PCR) is now widely used in malaria research for analysis of field samples. However, little has been reported regarding loss of sensitivity due to field methodology. Therefore, studies were carried out in relation to blood sampling (anticoagulants, culture medium, filter paper), storage (temperature, time and immediate lysis) and handling (repeated thawing and freezing). The PCR was unaffected by citrate and EDTA but partly inhibited by heparin (inhibition was reversed by heparinase at optimal concentrations). Samples collected on filter paper showed a significant 100-fold lower sensitivity (compared to control samples frozen immediately after collection) when stored at 30 degrees C and 60% humidity; and the paper quality appeared to be critical. Storage of unprocessed whole blood at 4 degrees C, 20 degrees C or 30 degrees C rarely resulted in any loss of sensitivity. Repeated thawing generally resulted in 10-fold loss of sensitivity compared to blood kept frozen until DNA extraction. The presence of antimalarial drug did not apparently affect sensitivity. We conclude that the mode of collection and storage of blood samples may influence the sensitivity of detection of malaria parasites by PCR. This may be critical in studies including individuals with low parasitaemia, mixed infections and comparison of data from different settings.

Parasites present in blood samples of asymptomatic carriers and in the midgut of mosquitoes collected within a few days from the same households, have been analysed by PCR. A high prevalence (32%) of infected mosquitoes was observed and, in half of these, two parasite species were found simultaneously. The distribution of parasite species in the mosquito correlated with that found in the infected persons. Genotype patterns of Plasmodium falciparum populations were however found to be different in the two sets of samples. These results and the potential of PCR are discussed with reference to investigations of the dynamics of malaria transmission.

Due to a strong background on Ecology acquired during my BSc degree, my MAIN AREA OF INTEREST has always been parasite epidemiology, especially malaria, transmission dynamics and interactions and ecological relationships between different parasites co-infecting the same host.
Within this scope, I have conducted several research projects in malaria endemic areas, namely Guinea-Bissau, Cape Verde, Mozambique and Equatorial Guinea (on-going project).
The unexpected observations in Cabo Verde led to a new line of research - human host factors of susceptibility/ resistance to malaria.
So far, encouraging results have been obtained and new plans and projects for future work are being implemented.
I have acquired EXPERIENCE in field work in endemic areas, both regarding blood and mosquito collections and at the laboratory, I mainly work with in vitro cultures, manipulation of experimental murine models, molecular techniques such as PCR-based techniques, cloning, sequencing and immuno-enzymatic techniques.
Also experienced in post-graduation training activities both locally and in endemic areas.
I am a full MEMBER of the CMDT-LA (Centre of Malaria and other Tropical Diseases, Institute of Hygiene and Tropical Diseases, University Nova of Lisbon) and Collaborator of CIAS (Centre of Research for Anthropology and Health, Faculty of Sciences and Technology, University of Coimbra) Also, several COOPERATION activities are carried out, which mainly fall on: 1) Advanced Training of Human Resources (eg PhD projects - Joana Alves, Ministry of Health of Cabo Verde, Fernanda Dias, Faculty of Medicine, University Agostinho Neto, Angola, Afonso de Almeida, Timor Lorosae; MSc projects - Juliana Miranda, Paediatric Hospital David Bernardino, Angola, Joana Alves, Ministry of Health of Cabo Verde); 2) activity integrated on projects (eg training of technicians in Chokwe, Mozambique, under the "Project to Integrated Malaria Control Program in Region of (Gaza)"; also in Mozambique, Faculty of Medicine, University Eduardo Mondlane (Maputo) with a) technology transfer for erythrocyte enzyme activity determinations, b) detection of the sickle cell trait by genotyping human HBB, c) detection of Plasmodium in peripheral blood from blood bank donors; in Cabo Verde, General Directorate of Health, when a mission was carried out (November/December 2007), involving two projects (POCI/SAU-ESP/55110/2004 and POCI/BIA-BDE/57650/2004) – an entomological survey was made and results reported to the General Directorate of Health – among others, these included a detailed description of the distribution of the vector Aedes aegypti, very relevant for the later dengue outbreak management in 2009.
